In this text, I try to describe the problem domain.<\/h3>\n\n\n\n<p>For now, let&#8217;s take psychotherapy and coaching as synonyms at a higher level. We are talking about the broad domain of healthcare now, including psycho-somatics.<\/p>\n\n\n\n<p><strong>Most straightforward answer: If the problem gets solved.<\/strong><\/p>\n\n\n\n<p>This is the conscious mindset in which most patients consult their physicians. It is also the mindset of the physicians.<\/p>\n\n\n\n<p>But what is &#8216;the problem&#8217;? I&#8217;ve been a practicing physician for years. At each consultation, it seems evident that &#8216;the problem&#8217; is pretty straightforward \u2015 until one lends some more time and attention to let &#8216;the real problem&#8217; emerge. The initial one has a role in this. Frequently enough, it is not the central role.<\/p>\n\n\n\n<p>Even more so in psychotherapy.<\/p>\n\n\n\n<p><strong>Three layers<\/strong><\/p>\n\n\n\n<p>In the goal of therapy, I see three possible conceptual layers. This distinction is equally relevant in psychological issues as in somatic ones.<\/p>\n\n\n\n<p>At each layer, the goal is bent on fewer symptoms, but from a different angle:<\/p>\n\n\n\n<ul><li>straightly reducing the symptom (&#8216;symptomatic&#8217; therapy)<\/li><li>removing the cause<\/li><li>growing beyond the symptom<\/li><\/ul>\n\n\n\n<p>In one diagram:<\/p>\n\n\n\n<figure class=\"wp-block-image size-large is-resized\"><img loading=\"lazy\" src=\"https:\/\/i1.wp.com\/aurelis.org\/blog\/wp-content\/uploads\/2021\/05\/symptom-three-1.jpg?resize=509%2C62&#038;ssl=1\" alt=\"\" class=\"wp-image-6108\" width=\"509\" height=\"62\" srcset=\"https:\/\/i1.wp.com\/aurelis.org\/blog\/wp-content\/uploads\/2021\/05\/symptom-three-1.jpg?w=760&amp;ssl=1 760w, https:\/\/i1.wp.com\/aurelis.org\/blog\/wp-content\/uploads\/2021\/05\/symptom-three-1.jpg?resize=300%2C37&amp;ssl=1 300w\" sizes=\"(max-width: 509px) 100vw, 509px\" data-recalc-dims=\"1\" \/><\/figure>\n\n\n\n<p>For instance, growing beyond the symptom in somatic medicine may be done through physical exercise, strengthening muscles and becoming more mobile again.<\/p>\n\n\n\n<p><strong>Each layer has a different view on &#8216;symptom.&#8217;<\/strong><\/p>\n\n\n\n<p>Using the above stratification:<\/p>\n\n\n\n<ul><li>The symptom is a nuisance to get rid of, so one can continue as before.<\/li><li>The symptom shows that some underlying cause needs to be found and treated towards a durable solution.<\/li><li>The symptom is a symbol and a call from deeper mental layers. It is a call for growth and should be listened to.<\/li><\/ul>\n\n\n\n<p>This also implies a different take on therapy.<\/p>\n\n\n\n<p>Note at this point that, as to symptom relief, it is case-dependent which of the three is most efficient, also at short term, especially in complex issues.<\/p>\n\n\n\n<p><strong>Therapeutic choice<\/strong><\/p>\n\n\n\n<p>&#8216;Success in therapy&#8217; is dependent on a choice between these layers \u2015 or a combination of them. The three are not crisply delineated, yet this doesn&#8217;t make the distinctions less relevant.<\/p>\n\n\n\n<p>It is best to make this choice an explicit part of the striving for patient consent. Patients \u2013 and people in general \u2013 can be educated in valuing this consent.<\/p>\n\n\n\n<p><strong>Success in therapy is thus dependent on the layer.<\/strong><\/p>\n\n\n\n<p>Obviously.<\/p>\n\n\n\n<p>If the above choice remains implicit, comparing therapies as to their success is also less evident. There is an implicitness in the comparison. This by itself doesn&#8217;t mean that one choice is better than the other.<\/p>\n\n\n\n<p>But the choice should be explicit. An implicit choice is also a choice, only one that hasn&#8217;t been thought through in-depth. Not each patient is to be burdened with choosing at each consultation. But at least, the caregiver should be aware of it and talk about it when appropriate.<\/p>\n\n\n\n<p>Also, and at least as important, it should be culturally on the agenda.<\/p>\n\n\n\n<p>If we want a Compassionate society, then the third choice becomes more relevant.
