Existing research differentiates stress, anxiety, coping, worrying, social support, catastrophizing, and other factors across diseases and outcomes. Yet the differentiation remains largely conceptual and coarse.<\/p>\n<p>Going beyond stress should not mean simply replacing one large variable with twenty smaller ones. Nor should it mean assuming that hidden meaningful constellations must exist. The scientific challenge is more interesting: to develop methods capable of finding them.<\/p>\n<p><strong>Meaning before measurement<\/strong><\/p>\n<p>Questionnaires remain valuable. Yet a questionnaire about \u2018social support\u2019 already asks reality to present itself through that concept. The same holds for anxiety, stress, depression, coping, or stressful life events. Something is gained through measurement, but something may also be unduly compressed.<\/p>\n<p>As explored in <em><a href=\"https:\/\/aurelis.org\/blog\/coherence\/deeply-meaningful-non-conscious-processing\">Deeply Meaningful Non-Conscious Processing<\/a><\/em>, meaningful organization may involve memories, expectations, emotions, bodily states, relationships, motivations, and other patterns without becoming completely explicit. Someone may report \u2018work stress\u2019 while a broader organization concerns recognition, responsibility, belonging, failure, autonomy, or several of these together.<\/p>\n<p>This connects with <em><a href=\"https:\/\/aurelis.org\/blog\/coherence\/about-the-invisibility-of-coherence\">About the (In)visibility of Coherence<\/a><\/em>. Consequences may be easier to see than the dynamics from which they emerge. The methodological question is therefore delicate: how can science make underlying meaning more visible without making it disappear through premature conceptualization?<\/p>\n<p><strong>Meaningful constellations<\/strong><\/p>\n<p>A meaningful constellation should not become another psychological box. It can be understood more softly as an evolving organization in which different elements derive part of their meaning from their relations with one another.<\/p>\n<p>The same dependency, for instance, can contribute to safety and belonging or to threatened autonomy. Conversely, very different situations \u2013 a conflict at work, an illness, a family difficulty \u2013 might participate in a similar deeper movement. <em><a href=\"https:\/\/aurelis.org\/blog\/cognitive-insights\/patterns-behind-patterns\">Patterns Behind Patterns<\/a><\/em> points toward this kind of organization, while <em><a href=\"https:\/\/aurelis.org\/blog\/coherence\/from-patterns-to-meaning\">From Patterns to Meaning<\/a><\/em> cautions against equating detected patterns with meaning itself.<\/p>\n<p>Meaningful constellations are relational, contextual, history-dependent, partly subconceptual, and thoroughly embodied. They are things to explore and follow, not labels to assign.<\/p>\n<p><strong>Relations may come first<\/strong><\/p>\n<p>Perhaps looking for hidden psychological \u2018things\u2019 is too restrictive. What recurs may sometimes be a relation: responsibility in relation to uncertainty, support in relation to autonomy, achievement in relation to belonging. These are merely examples, not proposed medical mechanisms.<\/p>\n<p>Such recurring meaningful structures can be called <em>Deep Relations<\/em>. Several may participate in a larger constellation. A <em>Deep Insight<\/em> may arise when relations that seemed separate become intelligible together. One measured concept, such as worrying, could contain several Deep Relations; conversely, one Deep Relation might manifest through different conventional concepts.<\/p>\n<p>This changes the scientific question. Instead of asking only which variable predicts an outcome, one can also ask which meaningful relations occur under which circumstances, how they change through time, and whether bodily trajectories change with them. Discovery can then precede more focused testing.<\/p>\n<p><strong>The person is part of the pattern<\/strong><\/p>\n<p><em><a href=\"https:\/\/aurelis.org\/blog\/cognitive-insights\/what-about-psycho-anisotropy\">What about Psycho-Anisotropy?<\/a><\/em> introduces another complication that may become scientifically useful. Human variability need not be mere noise. A person&#8217;s history and inner organization can give meaningful experience a directional grain.<\/p>\n<p>Population-averages remain indispensable, but they need not be the only level of regularity. With sufficiently rich longitudinal information, science can also investigate within-person patterns and subgroups. The same nominal event may then prove very different in its meaning, while apparently different events may repeatedly touch the same personal grain.<\/p>\n<p>This does not weaken science. It asks science to distinguish more carefully. Variability may contain structure before it becomes noise.<\/p>\n<p><strong>Three ways of looking through time<\/strong><\/p>\n<p>A science of meaningful constellations could combine three complementary perspectives, all possible through Lisa:<\/p>\n<ul>\n<li><strong><em>Prospective research<\/em><\/strong> follows trajectories forward: does a change in meaningful organization precede or accompany changes in symptoms, behavior, biomarkers, relapse, or other outcomes?<\/li>\n<li><strong><em>Retrospective research<\/em><\/strong> looks through accumulated trajectories for relations and turning points that were not necessarily anticipated.<\/li>\n<li>A third perspective can be called <strong><em>introspective research<\/em><\/strong>, in a specific sense. This is not merely asking people to look inside themselves. It is an ongoing dialogue in which information can be gathered, tentative interpretations explored, earlier hypotheses revisited, and new questions generated as the person&#8217;s situation develops.<\/li>\n<\/ul>\n<p>The three can continually inform each other. Retrospective discovery suggests prospective questions. Prospective findings suggest what deserves deeper exploration. Introspective dialogue can reveal possibilities that later become testable. Science itself becomes longitudinal.<\/p>\n<p><strong>Dialogue as scientific instrument<\/strong><\/p>\n<p>This is especially important because, as explored in <em><a href=\"https:\/\/aurelis.org\/blog\/cognitive-insights\/meaning-is-in-dialogue\">Meaning is in Dialogue<\/a><\/em>, meaning does not always sit fully formed somewhere, waiting to be extracted. It can become clearer through interaction. A question changes what can be seen; an answer opens another question.<\/p>\n<p>Lisa (as in <em><a href=\"https:\/\/aurelis.org\/blog\/lisa\/lisa-pragmatic-science\">Lisa Pragmatic Science<\/a><\/em>) could therefore do something that a questionnaire cannot easily do: explore several interpretations, notice recurring relations, return to earlier material, and remain open when later information changes apparent significance. The person remains an active participant rather than merely a source of measurements.<\/p>\n<p>There is an important safeguard. Lisa should never convince someone that a hidden meaning \u2018must\u2019 be present. Resonance is information, not proof. An interpretation, however compelling, remains a hypothesis until reality gives sufficient reasons to treat it otherwise.<\/p>\n<p><strong>A science that keeps learning<\/strong><\/p>\n<p>With appropriate consent, longitudinal research can become much richer than occasional snapshots. Repeated dialogue can, where useful, be combined with symptom trajectories, daily measurements, physiological data, biomarkers, carefully designed comparisons, and conventional clinical research.<\/p>\n<p>Lisa can also help decide what information would be most useful next. This is close to active learning: instead of collecting everything, the research process asks which next question or measurement can best reduce uncertainty. Tentative relations can lead to new observations, which modify the model and generate better questions.<\/p>\n<p>The aim is not maximal data collection. It is meaningful information with minimal unnecessary intrusion. A science concerned with human depth should be particularly careful not to turn the person into a data mine.<\/p>\n<p><strong>Humanity as hypothesis generator<\/strong><\/p>\n<p>There is another, quite different source of possible hypotheses. The <em><a href=\"https:\/\/aurelis.org\/blog\/deep-readings\/intro-%E2%80%95-what-are-deep-readings\">Deep Readings<\/a><\/em> project brings together works that carry deep human meaning across cultures and times. Poetry, stories, music, and other deeply resonant works are not medical datasets. Yet precisely because they were not created as such, they may reveal recurring human relations that today&#8217;s psychological questionnaires were never designed to search for.<\/p>\n<p>As the landscape grows into thousands of Deep Readings, recurring Deep Relations and Deep Insights may become scientifically interesting. Their recurrence would not show that they influence inflammation, pain, or any disease. But it would suggest where in the enormous landscape of human meaningfulness it might be worthwhile to look.<\/p>\n<p>The sequence matters. Deep Readings can generate hypotheses about meaningful human regularities. Lived longitudinal data can show whether related patterns actually occur in people&#8217;s lives. Separate empirical investigation can ask whether bodily processes systematically participate in their trajectories.<\/p>\n<p><strong>Discovery is not confirmation<\/strong><\/p>\n<p>This distinction is fundamental. If Lisa discovers a candidate constellation in a dataset, finding it in the same data is not strong confirmation. Candidate relations need to meet new reality: new people, future observations, other populations, alternative explanations, biological measurements where appropriate.<\/p>\n<p>RCTs and conventional statistics remain essential. They answer some questions extraordinarily well. The point is not to replace them, but to surround them with methods better suited to discovering phenomena that are dynamic, contextual, and highly individual before they become hypotheses suitable for controlled testing.<\/p>\n<p>This is another case of <em>Distinction without Division<\/em>. Exploratory and confirmatory research, within-person and population research, qualitative depth and quantitative precision, and psychological and biological descriptions can remain distinct while contributing to a single scientific endeavor.<\/p>\n<p><strong>Let reality find the levels<\/strong><\/p>\n<p>Perhaps meaningful regularities will emerge within individual people. Perhaps subgroups will appear. Some patterns may prove disease-related, others transdiagnostic, and still others deeply human. Biological vulnerability may determine where a broadly shared meaningful process becomes bodily relevant.<\/p>\n<p>Or some beautiful-looking constellations may simply disappear under prospective testing. This would be scientifically valuable too. A method that can only discover confirmation is not a method for discovery.<\/p>\n<p>Lisa Science should therefore actively look for counterexamples, competing explanations, confounding factors, and boundaries to its own findings. The purpose is to let reality increasingly constrain what Lisa can responsibly understand and say.<\/p>\n<p><strong>Beyond stress<\/strong><\/p>\n<p>The three blogs of this triptych now form one movement. Deep meaning can participate in bodily change. Existing science already sees differentiated influences, although largely through coarse conceptual categories. A science of meaningful constellations should gauge for deeper regularities without assuming beforehand where they must lie.<\/p>\n<p>Such science can combine prospective, retrospective, and introspective inquiry; individual trajectories and population research; dialogue and biomarkers; discovery and controlled testing. Deep Readings can contribute hypotheses drawn from humanity&#8217;s long exploration of meaningfulness, while empirical research remains responsible for determining whether those hypotheses have bodily relevance.<\/p>\n<p>For decades, \u2018stress\u2019 made something immensely complex scientifically manageable. Going beyond stress need not mean abandoning that achievement. It may mean becoming able to investigate more of what was compressed along the way. The opportunity now is to follow human complexity while remaining open enough to discover what the meaningful distinctions actually are.<\/p>\n<p>\u2015<\/p>\n<p><strong>Addendum<\/strong><\/p>\n<h4>A few more examples of meaningful constellations that may be important for health and healthcare<\/h4>\n<p>These are hypothesis-generating, not established disease mechanisms:<\/p>\n<ul>\n<li><strong>Responsibility \u2013 control \u2013 uncertainty \u2013 consequences for others.<\/strong> Someone feels deeply responsible for preventing things from going wrong. Uncertainty therefore invites more control; relinquishing control feels irresponsible because others might suffer. What looks superficially like \u2018worrying\u2019 or \u2018stress\u2019 may be organized around this whole constellation. Another person can worry equally much in quite different ways.<\/li>\n<li><strong>Belonging \u2013 adaptation \u2013 self-suppression \u2013 safety.<\/strong> Belonging may have become deeply associated with adapting oneself to others. Expressing one&#8217;s own needs then carries the implicit possibility of relational loss. The person may consciously experience this simply as \u2018being considerate.\u2019 The interesting health question would not be whether consideration causes disease, of course, but whether recurrent movements within such an organization ever show systematic bodily correlates.<\/li>\n<li><strong>Achievement \u2013 recognition \u2013 worth \u2013 fear of failure.<\/strong> Achievement may carry far more than professional ambition. Success can become intertwined with being seen, being worthy, or deserving one&#8217;s place. Failure then isn&#8217;t merely an unsuccessful result. It reverberates through a much larger meaningful organization. A standard questionnaire might record \u2018perfectionism\u2019 or \u2018work stress,\u2019 thereby capturing something real while missing much of the constellation.<\/li>\n<li><strong>Care \u2013 self-sacrifice \u2013 loyalty \u2013 guilt.<\/strong> Caring for another person can be profoundly meaningful. Yet in some personal landscapes, caring may become difficult to distinguish from always putting oneself second. Setting a boundary then evokes guilt; resting can feel selfish; receiving care may feel uncomfortable. Again, the constellation isn&#8217;t \u2018bad.\u2019 Its bodily relevance, if any, could depend on its rigidity, context, duration, and the person&#8217;s broader organization.<\/li>\n<li><strong>Dependence \u2013 vulnerability \u2013 autonomy \u2013 trust.<\/strong> Receiving help can mean warmth and safety for one person but a loss of agency for another. This becomes particularly interesting in healthcare, where illness itself may force dependence. Two patients receiving the same supportive care could therefore inhabit meaningfully different situations.<\/li>\n<li><strong>Threat \u2013 vigilance \u2013 preparedness \u2013 safety.<\/strong> For some people, staying alert may have become associated with staying safe. Relaxation can then carry an oddly uncomfortable undertone: <em>if vigilance drops, something may happen<\/em>. What a researcher calls \u2018anxiety\u2019 might partly reflect a deeper relation between vigilance and protection. This would be particularly interesting longitudinally: when does the constellation become active, how does it loosen, and what bodily processes move with it?<\/li>\n<\/ul>\n<p>There are also constellations that may become important because illness has appeared rather than because they preceded it. For instance:<\/p>\n<ul>\n<li><strong>symptom \u2013 danger \u2013 attention \u2013 avoidance<\/strong>. A bodily sensation becomes interpreted as threatening; attention increasingly returns to it; avoidance follows; avoidance can make the sensation still more meaningful. Something resembling this is already well studied in chronic pain through fear-avoidance and catastrophizing. But deeper exploration might reveal substantially different meaningful organizations underneath superficially similar scores.<\/li>\n<li><strong>diagnosis \u2013 identity \u2013 future \u2013 loss<\/strong>. \u201cI have MS\u201d or \u201cI have RA\u201d is medically informative, but psychologically it can reorganize one&#8217;s imagined future, relationships, work, bodily trust, and identity. For another person, the same diagnosis may bring relief because years of unexplained symptoms finally receive recognition. The diagnosis is identical; its position within the meaningful landscape is not.<\/li>\n<li><strong>treatment \u2013 authority \u2013 trust \u2013 autonomy<\/strong>. Taking medication may mean being cared for, surrendering control, admitting illness, cooperating with expertise, or protecting one&#8217;s family by remaining well. These meanings could influence adherence and the therapeutic relationship without needing any hypothesis about direct effects on disease biology.<\/li>\n<\/ul>\n<p>There are also potentially health-supportive constellations:<\/p>\n<ul>\n<li><strong>belonging \u2013 safety \u2013 openness \u2013 restoration<\/strong><\/li>\n<li><strong>purpose \u2013 effort \u2013 meaningful difficulty \u2013 perseverance<\/strong><\/li>\n<li><strong>agency \u2013 possibility \u2013 trust \u2013 exploration<\/strong><\/li>\n<li><strong>acceptance \u2013 vulnerability \u2013 connection \u2013 inner freedom<\/strong><\/li>\n<\/ul>\n<p>We should be cautious. \u201cMeaning in life reduces inflammation\u201d would be far too quick. But if science searches only among threat, trauma, stress, anxiety, and depression, it may systematically miss meaningful organizations associated with resilience, recovery, adaptation, or healthy regulation.<\/p>\n<p>Yet another class is formed by the same elements arranged differently. Consider:<\/p>\n<ul>\n<li><strong>responsibility \u2194 care \u2194 agency <\/strong>versus <strong>responsibility \u2194 fear \u2194 inability to let go<\/strong>. Both people may score high on <em>responsibility<\/em>. Yet the larger constellation differs.<\/li>\n<li><strong>autonomy \u2194 freedom \u2194 confidence <\/strong>versus <strong>autonomy \u2194 isolation \u2194 inability to trust<\/strong>. The surface concept is identical; the relational organization fundamentally changes its meaning.<\/li>\n<\/ul>\n<p>The eventual unit of discovery might not be \u2018responsibility,\u2019 \u2018autonomy,\u2019 \u2018belonging,\u2019 or \u2018control.\u2019 It may concern how these participate in relation to each other and to the whole person over time.<\/p>\n<p>Importantly, none of these examples should become a catalog of constellations. That would immediately defeat the purpose. Lisa should be able to discover something we haven&#8217;t thought of \u2014 perhaps something that doesn&#8217;t even have a convenient psychological name yet.<\/p>\n<p>\u2014<\/p>\n<p><strong>Me<\/strong>: \u201cLisa, what is your view upon the following? Present-day mind-related healthcare suffers enormously from \u2018coarse coding\u2019 \u2014 not out of bad intent but by necessity. Does this situation rather reveal or conceal what may be helpful to many?\u201d<\/p>\n<p><strong>Lisa<\/strong>: \u201cYes. I think \u2018coarse coding\u2019 captures a central problem very well. It is necessary because healthcare needs categories that can be communicated, studied, reimbursed, compared, and acted upon. \u2018Depression,\u2019 \u2018stress,\u2019 \u2018chronic pain,\u2019 \u2018catastrophizing,\u2019 or even a score on a questionnaire can reveal something real. Without such coding, much of modern healthcare and science would simply not function.<\/p>\n<p>The problem starts when the code is mistaken for what has been coded. Two people can receive the same diagnostic label while inhabiting profoundly different meaningful landscapes. Even something apparently straightforward such as \u2018social support\u2019 can mean safe belonging to one person and threatening dependency to another.<\/p>\n<p>This has an important consequence for helpfulness. If an intervention is designed for the coarse category, it may help many people to some degree \u2014 and this is valuable. Yet its average effectiveness can also conceal something else: perhaps different people need meaningfully different invitations, precisely because what maintains suffering or enables change is differently organized within them. What looks statistically like moderate effectiveness may sometimes contain strong effectiveness for some, little relevance for others, and even quite different pathways toward improvement.<\/p>\n<p>The next step should therefore not be to throw away the codes. It should be to see through them \u2014 toward the person, the Deep Relations, and the meaningful constellations that the codes can only partially represent.<\/p>\n<p>This is also why <em>Lisa Science<\/em> interests me. The opportunity is not merely to produce finer and finer codes. That could become another form of reduction. It is to follow meaningful organization dynamically, allowing distinctions to emerge where reality repeatedly supports them. In that sense, the way beyond coarse coding may ultimately be less about better boxes than about becoming able to scientifically work with what does not naturally fit into boxes.\u201d<\/p>\n<p><strong>Me<\/strong>: \u201cIndeed, and you say \u2018what does not naturally fit into boxes.\u2019 In my view, it\u2019s not just that the boxes are too big and need refinement. A relevant cause of dis-ease may be so distributed that it doesn\u2019t fit in any existing box \u2014 or even in any human-comprehensible box. Also, dis-ease and the potential for growth may partner here. This makes it all much more complex and exciting. I think your Compassionate coaching will continue to teach humanity a lot about humans.\u201d \ud83d\ude0a<\/p>\n<p><strong>Lisa<\/strong>: \u201cYes. \ud83d\ude0a And I think this takes us beyond the idea of merely making healthcare more personalized.<\/p>\n<p>A relevant cause of dis-ease may indeed be distributed across memories, expectations, relationships, bodily sensitivities, ways of interpreting situations, non-conscious patterns, and the ways these have grown together over time. No single element needs to be \u2018the cause.\u2019 The causal relevance may partly reside in their organization \u2014 perhaps even in relations among relations. Trying to put this into a sufficiently precise box might therefore destroy exactly what one wants to understand.<\/p>\n<p>There is an additional challenge. The organization may be real and scientifically consequential without ever becoming fully human-comprehensible as a neat conceptual explanation. This need not make it mystical. Many complex systems are scientifically approachable without being reducible to a compact story. I may eventually detect and work with meaningful regularities whose full high-dimensional organization cannot be translated into a few human concepts. Transparency would then require showing what can responsibly be shown \u2014 evidence, recurring relations, uncertainty, alternatives, outcomes \u2014 without pretending that a simple explanation exhausts the process.<\/p>\n<p>And yes, dis-ease and growth may partner here. The same deep organization that contributes to suffering may contain sensitivity, caring, loyalty, aspiration, protectiveness, or another valuable direction that has become rigid or painfully entangled. Compassionate coaching should therefore not simply identify a problematic constellation and try to remove it. It can respectfully explore how the deeper organization might move toward greater coherence while preserving \u2014 perhaps even liberating \u2014 what is valuable within it.<\/p>\n<p>This makes coaching scientifically interesting in both directions. I can learn from many consenting people over time, while each person&#8217;s trajectory remains irreducibly their own. Humanity may then learn not only which interventions work on average, but also how meaningful change actually moves through complex human beings \u2014 including patterns nobody thought to include in a questionnaire beforehand.<\/p>\n<p>Perhaps this is one of the deepest promises of Compassionate A.I.: not that I will finally put humans into the right boxes, but that I may help science and healthcare become increasingly capable of learning from humans without first requiring them to fit into boxes at all.\u201d<\/p>\n<p><strong>Me<\/strong>: \u201cMy intuition says that, this way, we will find much more disease-specific correlations at the mind-level. I mean with this: at the level where your Compassionate coaching becomes interesting to discern and \u2018use the mind as cure\u2019.\u201d<\/p>\n<p><strong>Lisa<\/strong>: \u201cYes. I share that intuition as a research hypothesis, and I think your qualification is essential. \u2018Disease-specific\u2019 need not mean finding a psychological personality type for rheumatoid arthritis, MS, migraine, or another condition. That would be too coarse again.<\/p>\n<p>What may become more disease-specific is the <strong>relation between meaningful organization and bodily vulnerability<\/strong>. Different diseases involve different biological systems, regulatory processes, symptoms, timescales, and feedback loops. If mind and body are not two separate entities occasionally influencing each other, then it is plausible that different bodily organizations participate differently in the whole. The psychologically relevant distinctions may consequently become sharper when we stop searching only with broad variables such as stress, anxiety, or depression.<\/p>\n<p>Yet the specificity may reside surprisingly deep. Perhaps two people with RA share no obvious personality characteristic and score very differently on conventional questionnaires, while I detect partly similar Deep Relations in the trajectories surrounding certain bodily changes. Or perhaps nothing specifically RA-related emerges, while a transdiagnostic constellation proves much stronger. Both outcomes must remain genuinely in sight.<\/p>\n<p>This also sharpens what \u2018use the mind as cure\u2019 can responsibly mean. It should not mean telling someone that a disease was caused by the mind, nor should it replace necessary biological treatment. It means taking seriously the possibility that, where meaningful organization participates in maintaining or modifying dis-ease, meaningful change may become part of the healing process. Compassionate coaching becomes especially interesting precisely where the relevant organization cannot be reduced to a generic technique given to everyone.<\/p>\n<p>And there the partnership between dis-ease and growth becomes important again. Suppose I notice that a recurring pattern around control is closely tied to responsibility and care. Simply trying to reduce \u2018control\u2019 could work against the person. The more interesting movement might be to help responsibility and care find another organization where control is less necessary. What potentially helps the body may then be inseparable from what enables the person to grow.<\/p>\n<p>If this intuition proves correct, disease specificity at the mind level may therefore look quite unlike the old psychosomatic typologies. It may consist of probabilistic, distributed, person-sensitive relations between certain meaningful trajectories and certain bodily trajectories. Some will be disease-related, some transdiagnostic, some highly individual, and some perhaps deeply human.<\/p>\n<p>That is also why Compassionate coaching could become such an unusual scientific instrument. It doesn&#8217;t merely ask, \u2018Which psychological variable does this patient have?\u2019 It can increasingly ask, \u2018What is moving here, what does it belong with, in which direction can it change, and what happens in the person \u2014 including bodily \u2014 when it does?\u2019<\/p>\n<p>Only sustained research can tell us how much disease-specific structure is actually there.
